Chemical Health Assessments WHAT IS A CHEMICAL USE/ABUSE ASSESSMENT? The purpose of an assessment is to help the student, family and school determine the extent of the problem relating to the student's alcohol and/or drug use. The assessment also helps make an action plan for the student and his/her family. Note: A urinalysis test is not a substitu- tion for the assessment interview. A urinalysis may be requested as part of the assessment process. The assessment process usually takes from one to two hours and includes the following: • Student has individual interview with the agency assessor. • Parent(s) or family member meets With assessor to discuss concerns. --As a parent, your child’s school attendance, grades, and behavior will be valuable information for the assessor. • The assessor will request that you sign a Release of Information to contact the school (and others you may authorize) with assessment recommendations. |
